Colophon (f. 112r): סליק מצות המפוזרים בזוהר וסילקא ספר לחם מן השמים אשר הומסר לנו ע"י הרב המקובל... יצחק לורייא אשכנזי זלה"ה, פה פיורנצולה יום ה' כה לחדש תשרי שנת תצ"ט לפ"ק. Owner (f. ii recto) "questo santo ספר della Eccmo Sig. R. Simon Vita Fontanella, Fiorenza [=Fiorenzuola], Apro 5533 [=1773]." He is probably the Rabbi whose Hebrew name was שמואל חיים פונטנילה. Perhaps the scribe was Benjamin Isaac b. Judah Hayyim Fontanella who copied New York, JTS, MS 2406 in Fiorenzuola in 1739. On f. ii verso a former owner, possibly C.M. Horowitz according to MMW, wrote: "No. 72". Deinard, 1914. MMW.
• -  Work 1: Ff. 1r-28r: זה ספר תולדות האר"י זלה"ה זכותו יגן בעדינו פירוש תיבות Sefer Toledot ha-AR”I, the legendary biography of the 16th century kabbalist Isaac Luria Ashkenazi. Includes פירוש התיבות a glossary to the abbreviations used, written on the outer margins. Not employed or mentioned in the critical edition by M. Benayahu (Jerusalem 1967). Another exemplar of this work copied in Fiorenzuola in 1714 is found in LC Hebr. Ms 177.
• -  Work 2: Ff. 29r-95r: לחם מן השמים Lehem min ha-Shamayim, by Jacob Zemah. Begins: זה השולחן אשר לפני ה' אשר בו כל המצות וחסידות הנמצאים בספרים של הרב זלה"ה.
• -  Work 3: Ff. 95v-112v: מצות ופקודים הנמצאים בספר הזוהר ובתיקונים דרך אלפא ביתא Alphabetical list of commandments and other matters found in the Zohar and Tikkunei ha-Zohar with references to the foliation in a printed edition. Also copied after Lehem min ha-Shamayim in other manuscripts (e.g., London, British Library Add. 26937, Montefiore 340), or separately (Moscow, RSL, MS Guenzburg 250).
