These manuscript leaves were removed from the back of the Library of Congress copy of Menahem Recanati's Ta'amei ha-Mitsvot (Basel 1581) which contains marginal notes in the same hand, and the page references in this manuscript refer to that volume. The fragment attached to the manuscript was once apparently part of the front leaf of the Recanati book. The manuscript was copied from another manuscript which had some lacunae as the copyist indicated several times by writing כאן חסר. The copyist believed that this text complemented Recanati's Ta'amei ha-Mitsvot and he attributed it, or at least parts of it, to Eleazar b. Judah of Worms. The heading reads: אלו הן המלואי' הצריכי' למלאות את החסר בספר הלז, הועתקו מספר כ"י יסדו וגם חברו האלקי המחבר ורב האלקי ר' אליעזר מוירמישא זצוק"ל. In fact the manuscript includes the introduction and first part of Recanati's kabbalistic commentary on the prayers up to ה' ימלוך in the morning zemirot (ff. 1r-6r), followed by the kabbalistic commentary by the anonymous author of Sefer ha-Maflig beginning in the middle of the yozer and continuing until the end (ff. 6r-15r). The scribe realized that the continuation was not by Recanati and wrote in the margin: לפי דעתינו מכאן עד הסוף הם דברי המקובל רבינו אליעזר מוירמישא..... The same text combining the two commentaries is also found in several other manuscripts such as Vatican ebr. 528, MS Paris, BnF héb. 857, Milan, Biblioteca Ambrosiana α 102 sup. and Munich, BSB hebr. 112. On ff. 15v-17v the scribe copied other short kabbalistic pieces mainly on the Divine Name. At the end the scribe wrote that the manuscript he was copying from was defective and that the end was missing לא מצאתי עו' בהעתק. MMW.
