• -  原題:「成都唐慎微續證類，中衛大夫康州防禦使句當龍德宮總轄修建明堂所醫藥所提舉入內醫官編類聖濟經提舉太醫學臣曹孝忠奉勅校勘。」傅希〓序云:「本草刊於東省舊矣，模印既久，漸齮齕，不可辨，閱者病之。方伯施君篤臣愈久而愈失其真也，乃謀諸寮宷洎臬閫諸君，請為重梓。余善之，遂屬醫官時孟陽輩，細加校讐，間有字畫之襲訛者改正之，藥性之有忌者增註之，可省可用，或不可多用者更詳補之。神農本陶隱居唐本等注，原刊陰文白字，今易以陽文口字;其來自神農者，又特以圈為識別。」是此本稍有校正，故行欵與成化嘉靖諸本不同。又此本眉端有音註，亦為以前諸本所無。惜此本後印，文字漫漶，有不能辨識者。卷內有:「黃綸之印」，「黃養吾記」，「綸山居士」，「蟫隱廬所得善本」等印記。傅希〓(序)隆慶六年(1572)，吳從憲(序)隆慶六年(1572)，施篤臣(序)隆慶六年(1572)，商輅(序)成化四年(1468)，陳鳳梧(序)嘉靖二年(1523)，王積(序)嘉靖三十一年(1552)，項廷吉(序)，馬三才(序)嘉靖三十一年(1552).
